Our new patient forms can be downloaded and completed in the convenience of your own home.

Our goal is to offer state-of-the-art chiropractic care in a comfortable setting by blending the tradition of chiropractic with the latest technologies available in our industry. We are frequently reminded how “comfortable” our office “feels” and how thorough a job we do. We take pride in providing you with quality service!

Make Yourself at Home

You’ll find plenty of convenient off street parking with a handicap accessible ramp conveniently located at the rear of the building if needed. You will be greeted by our friendly staff when you enter the office. If you have completed your admitting paperwork, great! If not, after doing so you will be given a tour of the office to get acquainted with our facilities and staff. We will likely show you a short 10 minute video and then you’ll meet Dr. Sam.

We Explain Everything First

The purpose of your consultation with the doctor is to discuss your health goals and see if chiropractic is likely to help you. If it looks promising, a thorough examination is performed including evaluation with the ProAdjuster which will help identify the cause of your problem. Your first office visit will typically last 60 minutes.
